I would be extremely cautious about engaging this practice. Not only did I make an appointment four weeks out that when I arrived they said was not made, I personally delivered new patient paperwork to the front office staff two weeks early. They claimed they did not have it. Giving out that much personal information to an office that then looses it OR WORSE. Beware
