Horrible waste of time. They asked for $100 dollars up front to see a doctor (red flag) and then I got to the appointment set 2 months ago only to find out that they do facials and cosmetic stuff only. If you want to see a doctor go someplace else. Dishonest.

I have been going to Joy at FORME for numerous years now . She is amazing and does a great job. I have people ask all the time how my skin looks so great and I am over 55 now! The office staff is so friendly and sweet and willing to help. I recently had some botox and a little filler. I cant remember the doctors name however once again he was great and took the time I needed to explain everything so I felt comfortable.

The staff was very nice and professional. The appointment went well. Then I received a bill for almost $800. I was referred to this clinic by my primary physician. I was told they accepted my insurance plan. What they left out is the fact that they would bill the first visit as a consultation, not an office visit. That part of the bill alone was more than half the bill. I feel sick that I am stuck with this size of a bill just by going in for them to check out the veins in my legs. The insurance company tried calling to have them change it to an office visit. They would not. I feel I was mislead and or they could at least have told me that that is how the first visit would be billed. I am very angry and disappointed by this clinic. I feel ripped off. I will never go to this clinic again and will tell everyone to go somewhere else as well.

Disrespectful, unprofessional, the receptionist was horrible, I hade an appointment scheduled 3 weeks in advance, take half of my day off from work to be there on time, got there on time fill the paperwork make me wait 20 minutes, I finally ask the receptionist why was taking so long, she when back to ask with a big attitude tours me, came back take an other patient wile I was waiting for the responds just to tell me that it will take an other 20 minutes or more !!! And then add “ if you want to reschedule “ ridiculous.couldn’t wait to get out of there.
